    Who's Eligible?

    • Full-Time W2 Employees working 30+ hours per week
    • Full-Time Independent Contractors working 30+ hours per week
    • Employed Owners

    Who's Not?

    • Part-Time W2 employees
    • Part-Time Independent Contractors working less than 30 hours per week
    • Seasonal Employees

    Benefits are effective the 1st of the month following 30 days from full-time date of hire. It is your responsibility to report eligibility changes, whether new or terminating. These changes should be sent to [email protected].

    Your Business

    • Greyfinch does not file 1094-C/1095-C forms. It is your responsibility as the employer to file 1094-C/1095-C forms if you are considered an Applicable Large Employer by the IRS.
    • If you are deducting eligible benefits on a pre-tax basis, you should maintain a Cafeteria Plan document on file. If you need a template, please contact [email protected].
  • Premium Billing

    It is your responsibility as the employer to payroll deduct all premiums and remit payment in full for the total amount due for all benefits enrolled. You will receive one monthly bill for all monthly premiums for Health Advantage, Blue Cross Blue Shield, and Reliance Standard. You will be direct-billed separately for the Greyfinch Direct Dental Care plan. Invoices are distributed electronically and payments can only be made via ACH.
